Prevention Hub
The Marin County Department of Health and Human Services is coordinating prevention services and opportunities across all our Divisions and programs in a new “Prevention Hub” which can assist communities and organizations to take action on health issues.

Marin County Systems Integration
Health and Human Services (HHS) has contracted with ZiaPartners, Chris Cline, M.D. and Ken Minkoff, M.D. to assist in developing a system of care that is easy to access, welcoming, recovery oriented, integrated, trauma-informed and culturally competent. The Zia process is guided by a Steering Committee that includes many of our community partners as well as HHS staff. On-going training is offered for “change agents” to increase competency to engage with people with multiple issues. Both the Steering Committee and the “change agents” meet on a regular basis.
  • Systems Integration Goal: To develop a system of care that is welcoming, recovery oriented, integrated, trauma-informed and culturally competent in order to most effectively meet the needs of individuals and families with multiple co-occurring conditions of all types.

Health and Human Services Transition Planning Process
HHS is in the process of planning a transition to consolidate the Divisions of Mental Health, Alcohol and Other Drugs, with the Prevention Hub and Public Health Programs into a single Community Health Division. During the first half of the 2012, we will develop a roadmap to guide the transition process over the next several years.

Transition Planning Goal: To become an organization that provides all clients easy access to comprehensive, integrated health care services, mental health services, and substance use services.

C4Yourself
Marin County, Department of Health and Human Services, participates in a new statewide online application system that allows individuals to apply for benefits. The site (www.c4yourself.com) is a secured site that keeps all of your information private and safe. Individuals can apply for benefits, complete and submit their annual redeterminations/recertification’s, access their quarterly/mid-year status reports. Also, members of the public will be able to view the status of their case/benefits.

Marin Health and Wellness Campus
The innovative Marin Health and Wellness Campus brings a spectrum of services and clinics together in one inviting place. It is a result of local individuals, organizations, businesses, and government pulling together in the spirit of cooperation and neighborhood revitalization. The project involved the design and renovation of approximately 74,000 square feet in five buildings. A sixth campus building, located a few hundred feet away, houses a new branch of the Marin Community Clinics, and provides additional parking. Seagate Properties, prior owner of the five adjacent buildings, managed the renovation of the buildings.

The Campus is built to LEED-NC Gold standard and is designed around principles of sustainability and ecological health. It integrates state-of-the-art green building practices for design, construction, and site operation.

Marin Continuum of Care
The Marin CoC is a collaborative, coordinated and inclusive process for the planning and management of a range of resources to address the needs of families and individuals at risk of homelessness and those who are currently homeless in Marin. The CoC works with community organizations providing a range of services which include emergency, transitional, and permanent housing, homelessness prevention and other service resources as part of the CoC process.

Disaster Preparedness
Health and Human Services is committed to Disaster Preparedness. As a Department we meet monthly to discuss Department-wide planning initiatives including those from Public Health, Mental Health, Social Services and Aging and Adult Services. We work closely with the County’s Office of Emergency Services, Community Based Organizations and City and Town Governments to continuously review, update and exercise County-wide plans.
